The Permission for Medication Administration at School and Child Care is available at the following link Permission for Medication Administration. It should be used for things like tylenol, ibuprofen, cough drops and other over the counter medications that are not listed in care plans above. This form can also be utilized for prescription medications that are not listed above. This does NOT include Diabetes, G-tubes, J-tubes, Ostomies, Genitourinary, Anxiety, or tracheostomy.
